Job Description

Our DT client company is seeking 2 Case Managers for a project that needs immediate attention. The position is responsible for working with clients participating in Permanent Supportive Housing Program, coordinating/updating referral services, and providing input regarding assigned cases. Also responsible for maintaining client files in accordance with agency policies, inputting client information in a designated database and gathering and reporting program metrics.  The successful candidate will perform these duties in a satisfactory manner and in accordance with the client's policies, procedures and core values.

Principle Duties:
  • Assesses families and individuals for strengths and barriers, including employment, behavioral health, criminal history and substance use and refer household to appropriate levels of housing and rental assistance.
  • Assists families and individuals with establishing and meeting housing and related goals, and identifying appropriate steps  towards achieving goals, including modifying goals as needed, and evaluation of progress.
  • Carries a caseload of families & individuals, which may include home-based case management.
  • Maintains precise and accurate documentation of case management services, including client files and entries into the program databases.
  • Coordinates with a variety of outside providers, as necessary, to meet client’s needs and avoid duplications of services.
  • Provides community resource referrals for clients.
  • Performs other duties as assigned.
Minimum Qualifications
  • Associate Degree; or two (2) years of experience in lieu of education
  • One (1) year of experience in case management
  •  Valid Texas Driver’s License
SKILLS REQUIREMENTS
  • Experience conducting outreach and presentations.
  • Flexible and effective team player, able to adapt to changes and a busy workplace environment.
  •  Knowledge and experience with housing issues and available resources.
  • Experience and knowledge with the challenges of and resources for those at-risk of homelessness.
  • Knowledge of HIV/AIDS and related issues (HOPWA only), and poverty-related issues.
  • Ability to work with diverse communities.
  • Excellent organizational skills and an ability to self-motivate.
  • Excellent written/verbal communication and people skills.
  • Intermediate MS Office skills: including Word, Outlook, and using internet.
  • Ability to work under pressure, address multiple priorities and meet deadlines.
  • Ability to identify key issues in problem solving and implement effective and creative solutions.
